Because we provide services to health care professionals and their patients, we require that all of our contractors successfully complete a background check and comply with our ethical standards. To complete your application as an independent contractor, you will need to submit a signed copy of the following documents:

Consent for Background Check
Once your application has been accepted, we will return a signed copy of the Associate Agreement to you. At that time, in order to complete the hiring process, you will need to complete and submit the following documents:
Compensation Plan (Sales Associate / Practice Consultant)
Company Ethics Policy
Software Terms and Conditions
Privacy Policy
Business Associates Agreement
Acknowledgement of HIPAA Policies
Federal 1099
Federal W9 (including copy of Driver’s License)
Liability Insurance Application
Upon receipt of the above documents, we will provide you with training materials, company protocols, and schedule your first training session. We require that you have reviewed all of the training materials prior to your training session. Please come to the training session prepared. You will receive login credentials to use the software system during your first training session.
